1. PRE-APPOINTMENT
Please ensure you complete the Massage Medical Consent form at least 2 days before your appointment.

All bookings require completion of a Medical Consent Form. If there is a contra-indication or health issue indicated that may limit or prevent massage treatment, I will either phone you to discuss., or discuss with you on arrival. In many cases the massage treatment can go ahead, whilst avoiding areas of concern.
If a massage cannot be carried out due to a contra-indication, you will receive a full refund of any booking fee paid.
3. LATE ARRIVAL
Late Arrival may mean a shorter period of massage if another booking follows straight afterwards. If there is a significant loss of your booked time, we can discuss a suitable future appointment where you can return to complete your session.
4. CANCELLATION
If you need to cancel a booking, particularly at short notice ,please ring or message me ( 07758 225 711 ) to re-arrange a new booking. Please refer to the booking calendar for suitable date / time.
5. NO - SHOWS
Please phone or Text me ( 07758 225 711 ) to let me know what the issue was and how you would like to resolve the situation.
6. LIABILITY DISCLAIMER
By returning the Medical Consent Form or signing a hard copy you are agreeing to the following disclaimer:
"I state and agree that all the information in the Medical Consent Form is true and correct to the best of my knowledge and understand that Grantley Massage has no liability for any Health Complaints / injury "
7. POST MASSAGE SORENESS & FEEDBACK
In undertaking a Raynor deep tissue massage it should be noted that during and after the massage session some soreness may be felt in parts of the body, as long held deep tensions are released by the treatment. This is normal and should disappear, but In case you have concerns please phone or e-mail me with any feedback.
8. GPDR
The Medical Consent Form and post treatment notes are retained on the premises in a locked cabinet for a period of 3 years. Treatment notes are very helpful in supporting targeted follow on treatments for the benefit of the person being massaged:
